Exemption Request Forms

Transport Canada has mandated that air travelers must provide valid proof of vaccination. Exemptions are limited, but unvaccinated travelers may apply for an exemption by submitting an Exemption Request Form.

Please complete the form that best applies to your need for exemption from full vaccination status and submit to a minimum of 21 days before a flight.

Forms must be signed by the exemption applicant or, if the form is being submitted on their behalf, must be signed by both the submitter and the applicant. They must also be signed by either a Doctor of Medicine (M.D.), Nurse Practitioner (NP), or Commissioner of Oaths, depending on the nature of the exemption request.

Medical Exemptions

Please note:

  • Completed Exemption Request forms must be submitted a minimum of 21 days before a flight to
  • Foreign nationals entering Canada are not eligible for a Medical Inability Exemption.
  • Foreign nationals travelling on a domestic flight in Canada must have supplementary medical information from a Canadian Doctor of Medicine (M.D.). Foreign medical information will not be accepted.
  • You will not be able to check in for your flight until your medical exemption has been approved.
  • Medical or Religious Exemption forms will not be accepted at the airport. All requests must be directed to the Exemptions Division at the email address indicated on the form.
  • Medical Exemption requests will require an attestation from a Doctor of Medicine (M.D.), Nurse Practitioner (NP) licensed to practice in Canada.

Religious Exemptions

Please Note:

  • Completed Exemption Request forms must be submitted a minimum of 21 days before a flight to
  • Medical or Religious Exemption forms will not be accepted at the airport. All requests must be directed to the Exemptions Division at the email address indicated on the form.
  • Exemption requests based on a sincere religious belief would need to have been sworn in front of a Commissioner of Oaths.

If the Medical or Religious Exemption is Approved

If Canadian North approves the Medical or Religious Exemption, the passenger will receive an email confirming their Exemption and an official Exemption document that must be presented when checking in.

The passenger will need to submit a valid COVID-19 test to the check in agents prior to being checked in. A valid COVID-19 test can be either of the following:

  • A negative molecular* COVID-19 test result
  • This test must be performed no earlier than 72 hours prior to the scheduled departure time.
  • Proof of a previous positive test result taken between 14 and 180 days before the flight

The passenger’s negative test result evidence needs to be in English or French (or a certified translation to English or French) and must show:

  • Name of the person that received the test
  • Name of the government or non-governmental agency which administered the test
  • Brand/type of the test that was administered
  • Dates of administration of the test
  • In the case of molecular test, the result of the test

The passenger must also carry their proof of a valid COVID-19 test with them. They may be required to present their test results at any time.

The passenger will be required to complete the health questionnaire and symptom screening during check in and have completed their company health questionnaire. They will also be required to present their valid COVID-19 PCR test result.

All passengers are required to follow the current face mask policy.

If the Medical or Religious Exemption is denied

If the Medical or Religious Exemption is denied by the Exemptions Division, the passenger will receive a notice by email and will not be permitted to fly.

Canadian North’s decision to approve or decline any Exemption is final and not subject to appeal.
